FAMHP portal: Pharmacy register module

Pharmacists-in-charge of a public pharmacy and licence holders can register following changes in the Register of pharmacies:

• the opening, closure or reopening of the pharmacy;
• the merger or transfer of location;
• the transfer of the operating licence to another person;
• the change of address of the operating licence holder (in case of a natural person);
• the change of the operator;
• the change of the fantasy name or correction of the registered address;
• the registration or deletion of an adjoining or non-adjoining cadastral plot;
• the change of the pharmacy titleholder or the registration holder in charge;
• the opening or closure of a website selling medicinal products;
• the making or discontinuing magistral preparations as a subcontractor;
• the starting or discontinuing an automated individual medication preparation (IMP) activity;
• the updated contact details.

Hospital pharmacists-in-charge titleholders of a hospital pharmacy and legal representatives of healthcare institutions can register following changes in the Register of pharmacies:

• the opening or permanent closure of the hospital pharmacy;
• the change in the language of communication, the hospital name or plan;
• the modification of the hospital sites, the administrative and delivery addresses;
• the change of the pharmacist-in-charge or the registration holder in charge;
• the updated contact details.
